2004-2005 – Service Coordination Grapeview Pilot Project

This project was completed as a partnership between Mason Community Network , Mason Matters, Grapeview School District, and the service provider stakeholders. Two of the top priorities identified by the community were better service integration and assuring that families are getting access to basic services (housing, food, health care). As a follow-up to the 2003 Children, Youth and Family Summit, a Case Management workshop are organized from February 2004. Sixty-four case managers and direct service providers who do some level of care coordination came together to describe the ideal case management system that was able to serve all Mason County families. Common descriptors of the ideal system were identified. The conclusion was to find a way to pilot test the model, and figure out the logistics as the pilot project evolved.

Grapeview School District was selected as the pilot project site. It was willing to participate, small enough to manage, had data that suggested that there were needs, but informal conversations suggested that families did not connect into services (due to local and bi-regional affiliations).

The Mason Community Network funded a full-time Family Resource Coordinator position to serve the Grapeview community. Mason Matters provided the project supervision. The outcomes of the pilot project resulted in a successful Readiness to Learn grant that expanded the project to three more rural school districts.
