2007 - Community Review - Service Coordination

Mason County Network and Mason Matters can launched an effort to review the status of our current service delivery systems capacity to provide cross-agency care coordination. Many families receive services from concurrent programs, and there is evidence that we are missing some opportunities to deliver coordinated care for these clients. The Community Network system is funded by Family Policy Council, and one its main mandates to engage local communities in review processes that improve the way state programs and funds are structured at the local level. Mason Community Network is looking at service coordination as the focus of its Current Review Process.

Fourteen Focus Groups were conducted throughout the community from January to May 2007. Both service providers and community members participated in these focus groups. The Community discussions resulted in the following recommendations for service system improvements:

  1. Develop structure for multi-agency team meetings for concurrently served clients.
  2. Develop structure to ‘transition’ clients from one agencies to the next, with Readiness to Learn (RTL) staff providing majority of the follow-up support.
  3. Develop release of information processes and policies that are initiated at beginning of service, if appropriate.
  4. Non-RTL service provider identify local RTL staff during intake.
  5. RTL coordinate school-based information between school personnel and outside service providers.
  6. Develop system to keep community case managers current with program and staff changes.
  7. Whenever possible and as appropriate, RTL coordinate first meeting with referral agency at local site.
  8. Expand RTL capacity to address gaps in system, specifically Truancy Courts and ChildCare system.
